Statement on opening the account;

The copy of the certificate on state registration;

The copy of the references confirming tax registration of the physical person -entrepreneur (F. 4-ОPP);

The copy of the document confirming the registration of the physical person -entrepreneur in the relevant body of the Pension Fund of Ukraine;

Cards of signature samples; (are registered on the spot);

Certified copies by hand of the first four pages of the passport, the pages with the place of residence and copy of the reference on assignment of the identification number of tax payer;

Client's application form + statement of the opening the account of the physical person

The list of documents for opening of the accounts for the formation of the basic capital, for opening of the account to a separate subdivision as well as opening of the other accounts is stipulated by instruction №492.
